I hereby agree to the following:

1. I am participating in a consultation, class, course, workshop or programme during which I will receive information and instruction about yoga, Ayurveda, Vedic astrology and health. I recognise that the yoga practices may require physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.

2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any physical fitness or health related programme, including yoga. I represent and warrant that I have no medical condition that would prevent my participation in physical fitness activities. Should this change during the programme, I will inform the leader of the programme as soon as possible.

3. In consideration of being invited to participate in a yoga practice, I agree to assume full responsibility for any risks, injuries or damages, known and unknown, which I might incur as a result of participating in the programme.

4. In further consideration of being permitted to participate in the yoga classes, I knowingly, voluntarily and expressly waive any claim I may have against the instructor, the owner or the leaseholder of the building for injuries or damages that I may sustain as a result of participating in online workshops, courses or programmes.

5. The purchase of participation in a workshop, course or programme is for use by the purchaser only and registration information, recordings, course materials, practices, teachings and event materials should not be forwarded, shared, copied, distributed, reproduced, published or taught to third parties without the prior written consent of Kate Smith Jamison Ltd; doing so may hinder your own access, as entry is by prior registration only. Responsibility for the confidentiality and security of the workshop, course or programme registration information and materials issued by Kate Smith Jamison Ltd lies with the registrant. If you believe that a third party has obtained details in an unauthorised manner, please promptly notify Kate Smith Jamison Ltd.

6. Kate Smith Jamison Ltd offers Ayurvedic Practitioner, Vedic Astrology and Yoga teaching services and is not qualified to give medical, legal, financial, tax, accounting or any other professional advice. Should such professional advice be identified as being required as an addition to any programme, it is your responsibility to seek out appropriate advice from a suitably qualified professional and we shall have no liability to you for any losses, costs, liabilities or other expenses incurred by you.

Please remember that your health and wellbeing is your own responsibility. It is your responsibility to ensure that information about your health provided to Kate Smith Jamison Ltd (KSJ) is kept up to date, accurate and full.
